I have one of the insurance plans listed above and would like to use my insurance benefits. I have insurance, but I am not planning to use my insurance benefits and prefer to self-pay. I have insurance that is not listed above and would like to discuss out-of-network options. I plan to pay out of pocket and seek reimbursement from my insurance company using a superbill. I plan to pay out of pocket and do not intend to seek reimbursement from insurance. I do not currently have health insurance. I am not sure which option is best for me and would like to discuss this during the consultation.
